Netflix’s Stolen Heartbeats, originally titled En coma, moves unusually fast even by thriller standards. Its short-form episodes compress a medical conspiracy, a supernatural connection and several overlapping relationships into only a few minutes at a time. At the center is Anabel Roldán, who is left in a coma while people around her begin making decisions about her life without realizing she can still perceive what is happening. The only person who can hear her is Rafael, a nurse who has spent years believing that the unexplained voices he hears are symptoms of mental illness.

By the time Part 1 ends, Anabel has returned to consciousness, but waking up does not make her safer. The danger around her reaches far beyond one corrupt nurse or one illegal procedure. Her hospital, her family business and people she personally trusted are all connected in ways she never suspected. Rafael becomes the person most committed to exposing the truth, which also makes him one of the biggest threats to the people trying to keep the operation hidden.

Rafael Realizes Anabel Is Not Dead

Rafael has heard unexplained voices before, and those experiences once led him into psychiatric treatment. When Anabel starts speaking directly to him in the hospital, his first reaction is fear because he assumes his mind is failing him again. He tries to ignore her, but Anabel keeps returning and gradually proves that she knows details Rafael could not possibly invent. The realization forces him to consider something he has resisted for years: the voices may be real.

Anabel eventually explains that her physical body is alive in intensive care even though her consciousness appears separate from it. She also tells Rafael that somebody is trying to kill her. That warning sounds impossible at first, but Rafael begins noticing strange behavior around her treatment. The more he investigates, the more obvious it becomes that Anabel’s condition is being exploited by people who see her as medically useful rather than as a patient.

Rafael Stops Anabel’s Family From Letting Her Die

Anabel’s family is forced to confront whether keeping her alive still makes sense, especially when doctors present her condition as hopeless. Because Anabel cannot speak to them directly, Rafael becomes the only person capable of defending what she actually wants. Through their connection, she guides him toward an advance medical directive that changes the situation. Finding it gives Anabel more time and prevents decisions from being made without her consent.

The problem is that Rafael cannot explain how he knows any of this without sounding unstable. Every attempt to help Anabel strengthens the suspicion that he is imagining things again. His colleagues see a nurse becoming unusually attached to a comatose patient, while Anabel’s relatives have no reason to trust his claims. The audience, however, knows that Rafael is the only person who understands how much danger Anabel is really in.

Azucena Connects the Hospital to the Illegal Transplant Ring

The first clear sign of the larger conspiracy comes through Azucena. Anabel remembers a distinctive tattoo on the arm of a woman involved in suspicious activity around her body, and that clue eventually points Rafael toward the nurse. What initially looks like misconduct by one hospital employee soon becomes much more serious. Anabel is quietly moved toward an unauthorized transplant procedure without her family understanding what is happening.

Rafael follows the warning and discovers Anabel being prepared for surgery. Carlos and Fernando are present, and the situation makes it clear that the hospital contains a hidden network dealing in illegal organs. Carlos is visibly shocked when he realizes the patient on the table is Anabel. That reaction becomes important later because Carlos is involved in the operation, but Anabel is one person he does not want sacrificed to it.

Carlos Has Been Having an Affair With Anabel

Anabel’s romantic life adds another layer to the conspiracy. Carlos is not simply one of the doctors involved in her treatment; he has also been having a secret affair with her despite being married to Anabel’s half-sister, Lucía. Before the accident, Carlos imagines eventually leaving Lucía and openly building a life with Anabel. The coma interrupts those plans and changes how Anabel sees him.

Lucía eventually learns about the affair, turning the medical crisis into a family betrayal. Carlos becomes increasingly desperate to keep control over Anabel, particularly once he realizes how close she has become to Rafael. Anabel’s relationship with Rafael developed when she had no status, money or physical presence to rely on. Rafael knows the part of her that existed when nobody else could hear her, and that connection eventually becomes romantic.

Anabel eventually becomes convinced that remaining trapped between her consciousness and her physical body is no longer sustainable. Rafael remembers the idea that the boundary between body and spirit may become accessible when a person’s vital signs are pushed dangerously close to death. Together, they decide to attempt something medically reckless. Rafael uses anesthetic drugs to push Anabel’s condition toward a critical threshold.

Carlos arrives during the emergency and tries to revive her, believing Rafael has lost control of the situation. For a moment, Anabel appears to die, and Rafael’s actions seem impossible to defend. He administers epinephrine, and Anabel suddenly gasps and regains consciousness. The plan works, but it also gives Carlos exactly the evidence he needs to portray Rafael as unstable and dangerous.

Anabel Wakes Up and Defends Rafael

Carlos immediately tries to remove Rafael from the clinic after Anabel regains consciousness. From the outside, his argument sounds reasonable because Rafael secretly administered dangerous medication to an unconscious patient. Anabel changes everything when she confirms that Rafael acted because she asked him to. She also insists that the two of them were able to communicate while she was in the coma.

Her family struggles to understand that explanation, but Anabel knows details that make it impossible to dismiss Rafael completely. Once she begins recovering physically, she keeps him close and eventually makes him her private nurse. Their relationship becomes openly romantic, which deepens Carlos’ resentment. Rafael is now both Carlos’ romantic rival and the person most likely to expose the transplant network.

James Spencer Leads Them Back to the Conspiracy

The investigation expands when James Spencer becomes connected to the hospital and the Roldán family’s business. Spencer is a wealthy foreign patient seeking a heart transplant outside legitimate medical channels. His presence gives Rafael and Anabel a clearer picture of how the network works. Wealthy clients are being connected with illegal organs through people operating across both the medical and business sides of the scheme.

A confrontation involving Spencer also exposes Carlos more directly. Rafael and Anabel see enough to understand that Carlos has been lying about how much he knows. The investigation then begins pointing toward Anabel’s own father, Gustavo. For Anabel, that possibility is much harder to accept than the involvement of hospital employees she barely knows.

Gustavo Is Involved Because of a Dangerous Debt

Gustavo’s role comes from financial desperation rather than medical ambition. He has become trapped by debts to dangerous people and turns to the illegal organ operation as a way to generate money. What may have begun as a way to solve one crisis soon becomes something he cannot easily escape. He fears that exposing the scheme could bring retaliation against his entire family.

Silvia eventually understands enough to see how dangerous the situation has become. Instead of immediately exposing Gustavo, she starts thinking about how the family and its business can be protected. Anabel, meanwhile, is slowly discovering that her father’s financial choices helped create the danger surrounding her. The conspiracy is no longer something happening around the Roldáns; it is partly being sustained by them.

Carlos Turns Anabel Against Rafael

Carlos realizes that Rafael is becoming impossible to control, so he changes tactics. Rather than denying the conspiracy completely, he gives Anabel a version of events that contains enough truth to sound convincing. He admits that Fernando and others are involved in illegal transplant activity and claims that he himself was trying to uncover what was happening. He then adds the lie that matters most: Rafael is supposedly part of the network too.

The accusation works because Rafael has repeatedly acted outside normal medical rules. He secretly medicated Anabel, investigated hospital staff and knows things that are difficult to explain without referring to their supernatural connection. Anabel does not return to Carlos, but she becomes less certain about Rafael. Carlos therefore succeeds in damaging the one relationship that gives Anabel a reliable path toward the truth.

Part 1 Ends With Gustavo’s Stroke and Rafael in Danger

Everything catches up with Gustavo near the end of Part 1. The pressure surrounding the criminal network, his family’s suspicions and the possibility of exposure become overwhelming. He suddenly collapses and suffers a severe stroke. Rafael is present at the clinic and helps during the emergency despite the hostility directed toward him.

The final conversations between Carlos and Fernando make clear that the conspiracy is entering a more dangerous phase. Gustavo now knows too much and may no longer be useful, while Rafael has discovered enough to threaten everyone involved. Carlos has also asked Azucena to deal with Rafael, but she is no longer responding. Part 1 therefore ends with Anabel awake, Gustavo critically ill, Rafael targeted and the people behind the transplant network increasingly willing to remove anyone who becomes a liability.
