Netflix’s Stolen Heartbeats depends heavily on relationships because its short episodes leave little room for slow character introductions. Anabel’s medical crisis quickly connects a nurse who hears unexplained voices, a married doctor, a betrayed half-sister and a family hiding dangerous financial secrets. The organ-trafficking plot becomes increasingly complicated, but the emotional structure stays easy to follow because the central characters are tied together in very direct ways. Rami Herrera and Jerónimo Cantillo lead the series as Anabel and Rafael, with Julián Trujillo and Paola Moreno completing the central romantic and family conflict.
The supporting cast gradually expands the thriller beyond that core group. Gustavo brings the Roldán family into the criminal operation, while Azucena, Fernando and James Spencer expose how the illegal transplant network functions. The result is a cast built around two overlapping worlds: Anabel’s family and the medical system surrounding her. By the end of Part 1, those worlds are almost impossible to separate.

Rami Herrera Plays Anabel Roldán
Rami Herrera plays Anabel Roldán, the woman whose body becomes the center of the series’ medical conspiracy. Anabel survives a violent incident but falls into a coma, leaving her unable to physically communicate with the people around her. Her consciousness, however, appears able to reach Rafael. That connection becomes her only way to warn anyone that she is in danger.
Herrera effectively plays two versions of Anabel during the early episodes. Her physical body lies almost completely still in intensive care, while the version Rafael encounters is frightened, sarcastic, determined and increasingly desperate to understand what is happening. Once Anabel wakes, the role changes again because she becomes an active investigator rather than a trapped victim. She also has to confront the consequences of her affair with Carlos and the growing evidence against her own family.
Herrera has previously appeared in Colombian productions including Medusa, La sustituta, Distrito Salvaje and La ley del corazón. Her role in Stolen Heartbeats gives her a story that moves from supernatural drama into conspiracy thriller without abandoning the emotional side of Anabel’s recovery.
Jerónimo Cantillo Plays Rafael
Jerónimo Cantillo stars as Rafael, the nurse who becomes Anabel’s only connection to the outside world while she is in the coma. Rafael has heard unexplained voices before, and those experiences previously led him into psychiatric treatment. Because of that history, he initially assumes Anabel is another hallucination. His reluctance makes their eventual bond more convincing because he does not accept her presence easily.
Rafael gradually becomes Anabel’s protector, investigator and romantic interest. He searches for the advance directive that helps prevent her family from making decisions she does not want, identifies people involved in the transplant operation and ultimately risks his career to help bring her consciousness back into her body. His willingness to break medical rules repeatedly puts him in conflict with Carlos. That same behavior later makes Carlos’ attempt to frame him as part of the organ ring more believable to outsiders.
Cantillo is already familiar to many Netflix viewers from Rebelde, where he played Dixon. His other television work includes Pasión de gavilanes, Verdad oculta and Aníbal Sensación Velásquez. Rafael is also the character who keeps the supernatural element alive after the story begins shifting more heavily toward crime and medical conspiracy.
Julián Trujillo Plays Carlos
Julián Trujillo plays Carlos, a cardiologist whose relationship with Anabel creates one of the most complicated conflicts in Part 1. Carlos is married to Lucía, Anabel’s half-sister, while secretly having an affair with Anabel. Before the accident, he imagines eventually leaving Lucía and building a future with Anabel. The coma changes that relationship completely.
Carlos is also connected to the illegal transplant operation. He participates in the system, although his reaction when Anabel is mistakenly brought into a clandestine procedure shows that he draws a personal line when she becomes the intended victim. That contradiction makes him more interesting than a straightforward villain. He is willing to benefit from the same criminal structure that almost kills the woman he claims to love.
Rafael becomes Carlos’ rival both romantically and professionally. Anabel falls for Rafael, while Rafael’s investigation threatens to expose Carlos’ criminal activity. By the end of Part 1, Carlos is actively trying to convince Anabel that Rafael belongs to the transplant network. Trujillo has previously appeared in productions including Enfermeras, Sala de urgencias, Déjà Vu and Perfil falso.
Paola Moreno Plays Lucía
Paola Moreno plays Lucía, Carlos’ wife and Anabel’s half-sister. She begins the story unaware that two of the people closest to her have been betraying her behind her back. Discovering the affair changes her relationship with both Carlos and Anabel. Her anger is therefore much more personal than anything connected to the hospital conspiracy.
Lucía and Carlos are already struggling before the affair comes fully into the open. Lucía wants a future and family with him, while Carlos has emotionally moved elsewhere. Once she understands that Anabel is the other woman, the betrayal becomes impossible to separate from the larger family crisis. Her hostility toward Anabel continues even while everyone around them is dealing with the threat posed by the organ ring.
Moreno has previously appeared in Te la dedico, La ley del corazón, Manual para ninjas and La nieta elegida. Lucía gives the show an important emotional counterweight because not every conflict around Anabel comes from criminals or corrupt doctors. Some of the damage is the result of choices Anabel herself made before the coma.
Gustavo Is Anabel’s Father and Part of the Conspiracy
Gustavo initially appears to be a protective father suspicious of Rafael’s sudden importance in Anabel’s life. As Part 1 continues, his role becomes much darker. He is financially involved in the illegal medical operation and has allowed the family’s business interests to become connected with the transplant network. His actions place Anabel directly inside a criminal system she never knew her father was helping sustain.
Gustavo’s motivation comes from dangerous debt rather than simple greed. He owes money to people he fears and believes the organ operation is the only way to keep them from retaliating against his family. That does not make his choices harmless, but it gives them a more complicated motive. Part 1 ends with Gustavo suffering a severe stroke just as Anabel is beginning to understand the extent of his involvement.
Azucena, Fernando and James Spencer Expand the Organ Ring
Azucena becomes one of Rafael’s earliest links to the conspiracy. Anabel remembers a tattoo connected to the people moving her through the hospital, and that clue eventually points toward the nurse. Her role makes clear that the transplant operation depends on people inside the clinic who can quietly move patients and manipulate procedures. By the end of Part 1, Azucena has also become connected to the plan to remove Rafael.
Fernando represents another level of the medical conspiracy. He works closely with Carlos and understands that both Rafael and Anabel have learned enough to become dangerous. His conversations with Carlos show that the people behind the operation are increasingly willing to consider violence rather than simply concealment. James Spencer then reveals the international side of the scheme as a wealthy foreign patient seeking an illegal heart transplant.
Together, those supporting characters turn Stolen Heartbeats from a supernatural hospital drama into a much broader criminal thriller. Rafael’s ability may be what initially connects him to Anabel, but the people around them reveal that her coma was only the entry point into a far more organized operation.
