The first batch of Stolen Heartbeats ends after dramatically changing the nature of its central mystery. Anabel’s coma initially looks like the main problem, but by Episode 31, she is awake and able to investigate what happened to her. That turns out to be far more dangerous because the conspiracy around her reaches through the clinic, her family’s business and people she once trusted. Instead of asking whether Anabel will survive, Part 1 ends by asking how long she and Rafael can survive after learning too much.

Carlos becomes particularly important in the closing episodes because he is fighting on two fronts at once. He wants to protect his role in the illegal transplant operation, but he also wants Rafael removed from Anabel’s life. Those goals begin to merge as Rafael’s investigation threatens both Carlos’ freedom and his personal control over Anabel. Gustavo’s collapse then creates another problem for everyone involved in the scheme.

Why Does Anabel Begin Doubting Rafael?

Carlos understands that Rafael’s greatest advantage is Anabel’s trust. Rafael was the only person who could hear her while she was in the coma, and he repeatedly risked his own career to protect her. Carlos cannot easily compete with that emotional history, so he attacks Rafael’s credibility instead. He gives Anabel a story that contains enough truth to make the lie harder to detect.

Carlos admits that Fernando and other medical personnel are involved in illegal transplant activity. He also presents himself as someone who discovered the scheme and tried to expose it. Once Anabel accepts those parts of the story, Carlos tells her Rafael is involved as well. There is no evidence in Part 1 that Rafael belongs to the organ-trafficking network, which makes the accusation a deliberate attempt to isolate Anabel from the person most likely to uncover the truth.

Is Carlos Part of the Organ-Trafficking Network?

Carlos is not simply an innocent doctor who stumbled onto the scheme. His dealings with Fernando and James Spencer show that he has participated in the illegal transplant operation. His involvement becomes particularly clear when Anabel is mistakenly brought into one of the clandestine procedures. Carlos is horrified because he does not want Anabel harmed, but his reaction does not erase his role in a system willing to exploit other patients.

That contradiction defines Carlos throughout Part 1. He believes protecting Anabel somehow separates him from the worst people in the network. At the same time, he is willing to help sustain the very operation that nearly kills her. His jealousy toward Rafael makes him even more dangerous because he no longer distinguishes between protecting the conspiracy and protecting his place in Anabel’s life.

What Did Gustavo Do?

Gustavo’s involvement expands the conspiracy beyond the clinic. He has financial ties to the illegal medical operation and has allowed the Roldán family’s business interests to become connected with the network. His motivation is debt rather than medical profit. Gustavo owes dangerous people money and believes the operation is the only way to keep them from retaliating against his family.

That desperation explains why he participates, but it does not make him innocent. People are still being exploited and killed for their organs, and Gustavo knowingly helps sustain the system. The closer Anabel gets to the truth, the harder it becomes for him to protect her without exposing himself. By the end of Part 1, he is trapped between the criminals he fears and the daughter who is beginning to understand what he has done.

Does Gustavo Die in Episode 31?

Part 1 does not confirm Gustavo’s death. He collapses after the pressure surrounding the investigation becomes overwhelming and is rushed into medical care. Doctors determine that he has suffered a severe stroke. Rafael helps during the emergency even though Gustavo has never fully trusted him.

Carlos and Fernando later discuss Gustavo’s condition in a way that reveals how coldly they now view the situation. Gustavo has become a liability because he knows too much, but his death could also create new problems. Anabel is already suspicious, and losing her father could make her investigate the conspiracy more aggressively. Gustavo is therefore still alive when Part 1 ends, but his condition leaves his future uncertain.

Why Are Carlos and Fernando Afraid of Rafael?

Rafael has become the biggest threat to the operation because he refuses to stop investigating. He already knows about Anabel being moved toward an illegal transplant and has identified suspicious hospital staff. His investigation also leads him toward James Spencer and the connection between the clinic and the Roldán family’s business. He is getting close enough that Carlos and Fernando can no longer treat him as an irritating nurse.

Carlos first tries to destroy Rafael professionally. When that does not work, the conversation turns toward more permanent solutions. By Episode 31, Rafael is no longer simply being discredited; he is being discussed as someone who may need to be removed. That escalation is one of the clearest signs that Part 2 will become more dangerous than the first batch.

What Happened to Azucena?

Azucena remains one of the biggest unresolved pieces at the end of Part 1. Rafael identifies her early because of the tattoo Anabel remembers, and her role in moving patients makes her a direct link to the transplant network. She is someone Carlos and Fernando can use when they need actions carried out inside the clinic without attracting attention.

Carlos later reveals that he instructed Azucena to deal with Rafael. The problem is that she has stopped responding. Part 1 never explains whether she has disappeared, betrayed the group, been silenced by someone else or is preparing another move. Her absence is clearly positioned as a mystery rather than a forgotten subplot.

Does Anabel Choose Carlos or Rafael?

Anabel does not reunite with Carlos by the end of Part 1. Even after he manages to make her doubt Rafael, she understands that continuing the affair would mean betraying Lucía again. Everything that happened during the coma has also changed the way she feels about Carlos. He represents a life built around secrecy, while Rafael became important to her at the moment when she had no control over anything else.

Her relationship with Rafael is more complicated because Carlos succeeds in creating doubt. Anabel falls for Rafael because he believed her when nobody else could hear her, but the accusation that he might be connected to the organ ring unsettles her. Part 1 therefore ends with Anabel emotionally separated from Carlos but not completely secure in her trust of Rafael.

What Does the Part 1 Ending Set Up?

The ending leaves almost every important character in an unstable position. Gustavo has suffered a stroke and may no longer be capable of controlling the situation he helped create. Carlos and Fernando know the conspiracy is becoming harder to contain, while Rafael remains alive and determined to keep investigating. Azucena has disappeared at exactly the moment she was supposed to remove Rafael from the equation.

Anabel is also much harder to control now that she is awake. She knows her father is involved, she has encountered Spencer and she understands that the operation reaches through both her family and the hospital. The problem is that she still does not know which version of events is true. Part 2 therefore begins with the conspiracy exposed enough to be dangerous, but not exposed enough for Anabel to know exactly who her enemies are.
